How To Request Records

Clients’ clinical records are securely stored in the My Best Practice electronic records system, which is a HIPAA and PCI compliant electronic health record system with multiple layers of encryption to protect data and secure data servers monitored 24/7.

Clients have the right to access their records. Please complete and submit a Records Request form to Gina Leggio, LMFT, at 810 N. Dixie Dr., Ste. J, #364, Clute, TX, 77531 or through the My Best Practice client portal. Either a summary can be provided or a copy. The fee for making and mailing a copy is 25 cents per page. When requesting a copy, please allow 15 days to provide a copy. When requesting a summary, please allow 10 days from the request date unless the record is quite extensive, in which case please allow up to 30 days for a summary.

The laws and standards of my profession require that I keep treatment records for 7 years after the termination date, and for minors, 7 years after they turn 18.
